New Patient Registration
You can collect a registration pack from reception or print one off here New Patient Registration Pack
We are only able to provide you with urgent or emergency medical services until you have formally joined the practice. We try to keep the joining (or “registration”) process as simple as possible..
You do not need an appointment to register. Just come to reception at your most convenient surgery.
We will help you fill in a questionnaire about yourself, your health and medical history. This helps us to give you the best care.
We would then like you to book to have a brief check-up with our practice nurses. This includes blood pressure, weight and a urine test.
Once the registration form, questionnaire and check-up are done, we can register you with the health authority as having joined the practice. We will then be able to offer you the whole range of NHS health services.
If you have provided us with a mobile number we will occasionally send out text messages, please let us know if you do not wish to receive them.
We operate from two sites, both practices are accessible by wheelchairs to all clinical areas and both have disabled toilets.
Urgent appointments are available at both sites.
Guide to GP Services
The Royal College of General Practitioners has produced a useful guide for patients about the services on offer at GP Surgeries and how to access them. You can download the guide below.
